USE THIS FORM ONLY IF AND WHEN YOU AND YOUR TEAM MEMBERS HAVE ALREADY ENTERED AND PAID ON-LINE AS INDIVIDUALS.

This form is to be completed by the Team Manager. Teams consist of four swimmers of either sex and any eligible age. Categories are Open (swimmers aged 19 +), Junior (swimmers aged under 19), and Wetsuits. Ages are as at race day, March 21, 2010. This form must be lodged by midnight, Thursday, March 18, 2010.

THIS TEAM ENTRY FORM WILL NOT BE ACCEPTED UNLESS ALL SWIMMERS HAVE THEIR REGISTRATION ID NUMBERS LISTED FROM INDIVIDUAL ONLINE ENTRY.
